Below is a list of FORMS and PARENT RESOURCES that are available for you. We hope that you will use this online repository for registrations, keeping us up to date about your student, keeping track of how trips are going, and for getting friends and new students connected to our ministry programming!

First Student Ministries Telephone Hotline - Going on a Mission Trip? Want to know when they have arrived? Or when they are returning? Call the Student Information Hotline at 248-646-6407 and press "5". This will give you the latest information on what is happening. But remember, NO NEWS IS GOOD NEWS!!!

Medical Treatment Form - The same medical treatment form is used by the Student Ministry, the Children and Families Ministry, and the Music Ministry. Each youth must have a current medical treatment form on file with the appropriate ministry before participating in the events of that ministry.

STEPS: 1) Print the medical treatment form or obtain a copy of the form at the appropriate ministry office. 2) Fill out all of the requirement information. 3) Sign the form in the presence of a Notary Public. There are several Notaries at the church Monday-Friday. If you wish to sign the form at church, please call in advance to be certain a Notary will be available. 4) Return the form to the appropriate ministry office.

Prescription Drug Form - If a child attending any church-sponsored retreat or mission trip requires prescription medication on that trip, the prescription drug form must be turned in at the beginning of the event. The same form is used by the Student Ministry, the Children and Families Ministry, and the Music Ministry.

Peanut Allergy Reminder - Just a reminder that the church and all church events are PEANUT FREE. There are several kids in the student group who are severely allergic to peanuts, so PLEASE make sure any food and snacks you bring to church or on a trip or retreat are peanut free! This can be a matter of life or death for these kids, so please stop and think before you bring those Snicker bars to the retreat!
